These mid-year check-in worksheets can be used in a variety of very helpful ways:
a. As a communication to parents on how their child is doing in class (both behaviorally and academically)
b. As a personal gut-check for students to see how they are doing both behaviorally and academically and to challenge them to finish of the quarter/trimester/semester off strong
c. As a way for teachers to gain insight into a student’s plan for success and personal view of their own status and effort
d. As a sheet that students fill out and then you (the teacher) use during parent/teacher conferences shortly thereafter (having this sheet provides a number of helpful talking points when meeting with parents)
All documents provided are in easy-to-edit formats (Microsoft Word) so you can change titles, questions, etc. to your liking. Take a few seconds to make whatever adjustments you want and then print them off to use as early as tomorrow!
